Assisting individuals experiencing homeless in Des Moines, Iowa
The Continuum of Care Program is a US Department of Housing and Urban Development (HUD) Program designed to promote community-wide commitment to the goal of ending homelessness. The Continuum of Care disperses HUD funds to numerous programs in Polk County to accomplish this goal through Rapid Rehousing, Permanent Supportive Housing, and Transitional Housing.

Polk County offers a coordinated intake approach through community partner Primary Health Care. Coordinated Intake offers a centralized approach to addressing the needs of individuals and families experiencing homelessness. While completing an intake, clients complete an assessment to assist individuals and families in resolving their homelessness and may be assigned to a case manager, referred to shelter, or referred to other agencies for further assistance.
